NEW PATIENT FORMS
Your First Visit
Please plan on being here for 45-60 minutes. You can fill in the forms online by clicking the button at the top of this page. If you prefer to fill them out when you arrive, we recommend you get here 15 minutes before your appointment time.
Then, we’ll start a thorough chiropractic assessment consisting of:
- A spinal health check
- Postural Analysis
- Scans with the Insight™ Subluxation Station to measure nervous system health
- X-rays to locate any abnormalities
This examination procedure will help reveal the cause of your concern and give us a baseline for future progress assessments.

Your Second Visit
This appointment takes 30 to 45 minutes. After we’ve had time to review your examination findings and health history, we’ll make recommendations to assist you in attaining your goals. We call this visit the Report of Findings, where you’ll find out how we can help you, how long it’ll take, and what investment you’ll need to make.
Upon starting care, we’ll introduce you to the techniques we use for chiropractic adjustments. Dr. Mark and Dr. Barry have trained in several highly effective methods and uses them based on years of experience. We’ll answer all the questions you have before beginning.
Regular Visits
Each appointment builds on the previous, gaining the momentum we need to help you optimize your nervous system health. We’ll hold progress examinations along the way, making sure that you see the improvements you desire.
We know your time is valuable, and do everything possible to run on time and get you back to your busy schedule. Subsequent visits will take approximately 5-10 minutes.
